The direct answer
Why there is no single 'optimal' number
The honest answer is that the guidelines do not set one. The Endocrine Society's 2018 clinical practice guideline does not define an optimal estradiol target for men on testosterone therapy, and it does not recommend measuring estradiol routinely. Popular numeric targets circulate in forums, but they are not drawn from a validated trial and no single figure has been established as the goal.
Estradiol is not a byproduct to be minimized. In men it is made mostly by aromatization of testosterone, and it is required for normal bone, for aspects of sexual function, and for holding down fat mass. When testosterone rises on TRT, some of it converts to estradiol - that is expected physiology, not a problem to chase to zero.
The evidence that crushing estradiol is harmful
The clearest data come from a randomized dose-response trial in which healthy men had their own hormones suppressed and were then given graded doses of testosterone, half of them also taking an aromatase inhibitor (anastrozole) to block the conversion of testosterone to estradiol. When estradiol production was blocked, the percentage of body fat went up across the testosterone doses, and sexual desire and erectile function fell. That design is what lets the study attribute those effects to estradiol specifically, rather than to testosterone.
Bone tells the same story. The work on estrogen and the male skeleton reports that estrogen deficiency, not androgen deficiency, is the primary driver of bone loss in men, and that serum estradiol below roughly 10 pg/mL is undesirable for bone. A separate review adds that libido is impaired when estradiol is driven too low, and that completely eliminating estradiol, or lowering the testosterone-to-estradiol ratio too far, adversely affects sexual desire. The consistent signal across these sources is a downside to driving estradiol low, which is the opposite of treating a higher reading as automatically bad.
Aromatase inhibitors: what the guideline says
An aromatase inhibitor such as anastrozole lowers estradiol by blocking its synthesis. The Endocrine Society 2018 guideline names no estradiol target and does not include estradiol in its standardized monitoring plan, and the reviews above warn against over-suppression. Because the drug pushes estradiol down, the documented risk it carries is exactly the bone, libido, and body-composition harm described above. The 'sensitive assay' point
Before acting on any estradiol number in a man, the lab method matters. Men run much lower estradiol than women, and the Endocrine Society's position statement on estradiol measurement notes that such low concentrations are too low to be measured accurately or precisely by routine methods, and that different assays disagree with each other. The sensitive assay (liquid chromatography-mass spectrometry) is the method suited to the low male range. A standard immunoassay can read differently, which is why an unexpected result is often confirmed on a sensitive assay before anyone changes anything.

Treat any specific estradiol target you see quoted as a claim to check against the source, not a settled fact - the guidelines do not name one. What is well supported is the direction of harm: estradiol is essential in men, and suppressing it too far damages bone, blunts libido and erectile function, and raises body fat. What is the optimal estradiol level on TRT?
No single optimal number is established in the guidelines. Commonly cited community targets exist, but they are not from a validated trial, and the Endocrine Society does not define an optimal estradiol target or recommend routine estradiol monitoring on testosterone therapy.
