What SHBG does
Sex hormone-binding globulin (SHBG) is a carrier protein made mostly in the liver. Most of the testosterone in your blood is bound - a large part to SHBG, the rest loosely to albumin - and only a small fraction circulates free. Free testosterone plus the loosely albumin-bound portion is the bioavailable fraction, the part that can act on tissue. SHBG is the tight-binding carrier, so how much of it you have sets how your total testosterone splits between bound and free.
That single fact is why SHBG matters when you read a testosterone panel. Total testosterone counts everything - bound and free together. When SHBG moves, the total moves with it, even if the amount of testosterone actually reaching your tissues has not changed much.
Why low SHBG splits total from free testosterone
With less SHBG to bind it, a larger percentage of your testosterone is free - but there is also less carrier holding testosterone in circulation, so the total number drops. The result is a split: a man with low SHBG can show a low or borderline total testosterone while his free testosterone is normal.
This is not hypothetical. In one analysis, obese men had meaningfully lower total testosterone than non-obese men, yet their calculated free testosterone did not differ between the groups [2]. The low total was the SHBG talking, not a real testosterone deficiency. Hypothyroidism
Thyroid status influences SHBG; because a condition that alters SHBG changes how a total testosterone number should be read, the Endocrine Society advises reading free testosterone in that setting [1].

Low SHBG and injection frequency - what people report

Does low SHBG mean I have low testosterone?
