NAD+ injections vs NAD+ IV

The IV drip is what the wellness clinics sell; the subcutaneous shot is what a lot of people end up doing at home. This compares the two on the things that actually differ - cost and delivery - and the one thing that does not: the evidence, which is thin for both.

Short answer

If you are self-administering, subcutaneous is the accessible, far cheaper route - roughly $40-$290 per injection at a clinic and less on your own, versus $300-$800+ for a single IV session. The IV is faster, fully bioavailable, and supervised, which is worth something for the flush. But neither route has an outcomes trial behind the anti-aging, energy, or addiction claims, so the cost gap is not buying you proven benefit - it is buying you speed and a nurse.

The cost gap, in numbers

Clinic pricing pages put a single NAD+ IV session commonly between $300 and $800, with one clinic listing $429 for 250 mg, $549 for 500 mg, and $879 for 1000 mg in-clinic - and mobile or high-dose sessions running past $1,000, up toward $2,000 in major cities. The same clinics quote subcutaneous injections at roughly $40-$290 each. Self-administering from a compounded vial drops the per-dose cost further, which is the whole reason the subcutaneous route exists as a home practice.

On evidence, the routes are tied at zero

It is tempting to assume the pricier, clinical IV must be better supported. It is not. A 2026 systematic review of 113 studies found no outcomes trials of intravenous or intramuscular NAD+ for anti-aging or wellness at all. The IV route has slightly more human data in one narrow sense - the single pharmacokinetic study used a 6-hour IV infusion - but that study measured blood levels, not benefits. The subcutaneous route shows up only in a small unpublished tolerability preprint. So the honest comparison is: IV is better characterized in the blood, neither is proven to work, and the precursor pills (NR, NMN) that do have trial data are a different molecule that does not transfer to either injection route.

Is NAD+ IV better than a subcutaneous injection?

There is no head-to-head trial comparing outcomes between the two, so 'better' has no evidence-based answer for the marketed benefits. What differs is the delivery: an IV puts the full dose into your bloodstream immediately under supervision, while a subcutaneous shot absorbs more slowly, costs far less, and can be self-administered. Neither route has an outcomes trial showing it improves energy, slows aging, or treats addiction.

Can you inject NAD+ at home instead of getting an IV?

People do - subcutaneous NAD+ is the self-administered route, dosed by volume from a made-up solution. But it is a compounded product with no proven effective dose and no manufacturer safety standard, so sterility and purity are not guaranteed, and the same molecule that causes the IV flush can cause chest tightness and nausea if injected too fast. This is a decision to make with a licensed provider, not on your own.

Why is NAD+ IV so much more expensive?

You are paying for chair time and clinical overhead. NAD+ has to be infused slowly - often over one to several hours - to keep the flush tolerable, which means a nurse, a room, and monitoring for each session. A subcutaneous injection needs none of that, which is most of why it costs a fraction as much.
