There is no single answer - the benefits arrive on different timelines. In the most-cited review of onset times (Saad 2011), sexual interest improves within a few weeks (about 3 weeks, plateauing at 6), mood and quality of life shift within weeks to a few months, red blood cells and PSA change over months, fat and muscle move across 3-12 months, and bone density and glycemic effects take up to a year or more. The short version: weeks, months, and a year-plus

Saad et al. 2011 pooled the published data on when each effect of testosterone treatment begins and when it reaches its maximum. The headline is that the benefits do not arrive together. The fastest effects - sexual interest, mood, and insulin sensitivity - are measured in weeks. The body-composition and bloodwork effects run over months. Bone is the slowest, still building past a year. Reading the timeline this way sets a realistic expectation: the first few weeks are not the whole story, and the effects you can measure early are not the effects that take longest.

What takes a year or more

Bone is the outlier. Saad 2011 reports that effects on bone are detectable already after 6 months and continue for at least 3 years - the longest arc of any effect in the review. So a bone-density benefit that is not visible at the six-month mark is not necessarily absent; it is on the slowest of these timelines. The body-composition gains also keep creeping upward past the one-year stabilization point.

Why the monitoring window matches the timeline

The Endocrine Society guideline recommends checking symptoms, hematocrit, and PSA over the first months of therapy and periodically after that. That window lines up with the biology in the Saad review: red blood cells peak at 9-12 months and PSA plateaus around 12 months, so a single early reading does not capture where those markers settle.
