The direct answer
It depends on the brand, and every weekly label gives a window. Semaglutide (Wegovy, Ozempic): take the missed dose if it's within 5 days of when it was due, which is the same as the next dose being more than 2 days away. Otherwise skip it. Tirzepatide (Zepbound, Mounjaro): take it within 4 days (96 hours), otherwise skip it. Either way, go back to your usual day. Never double up.

Why the numbers look different but aren't
Wegovy counts forward to your next dose, and Ozempic counts from the dose you missed. On a weekly schedule it's the same rule: 7 days minus the 2-day minimum gap leaves a 5-day window. Tirzepatide's labels ask for at least 3 days between doses, which is why its window is 4 days.
Missed two or more weeks
Only the Wegovy label spells this out. If 2 or more consecutive doses are missed, it says to restart the dose escalation at a lower dose, to reduce the risk of the stomach side effects that can come with jumping back in. The Ozempic, Zepbound and Mounjaro labels give no rule for several missed weeks. Ask your prescriber before going straight back to your last dose. Compounded semaglutide or tirzepatide
Compounded products have no FDA label of their own. The brand labels above are the closest reference for the same molecule, but the instructions from your prescriber or pharmacy come first, especially because compounded vials are dosed in syringe units rather than a fixed pen dose. Can I take two doses to catch up?
No. Every label says to skip a dose that's outside the window and carry on with your usual schedule.
I'm 3 days late on Ozempic. Is it too late?
No. Ozempic allows up to 5 days after the missed dose. Take it now, then go back to your usual day.
I'm 5 days late on Zepbound. What now?
That's outside tirzepatide's 4-day window. Skip it and take the next dose on your usual day.
Can I just move my injection day permanently?
Yes, if the gap between two doses is at least 2 days for semaglutide or 3 days for tirzepatide.
